Yes, I have. In college, I once had on an outfit that was very low cut in the front. I was leaning forward to pay attention in class and I looked down and saw that my breasts spilled out of my top. I looked around the class and realized only one person had seen me before I pushed my boobs back into my top. I also had another incident where I was wearing a long skirt and it was caught in my undies and I was walking around with my butt out.
